Contractor Induction Form

The contractor induction form is the acknowledgement: the signed record that the contractor received the HSE induction, understood the site rules, received the PPE, and knows the emergency. It is the document that goes in the file, and the one the insurer and the investigation ask for after the event.

It is the counterpart to the induction checklist: the checklist is the walk-through at the gate, the form is the acknowledgement and the sign-off. The contractor signs that they received the induction and understood the rules, the inductor signs that they gave it, and the file reference records where the induction sits.
