PPE Issue Checklist

The PPE issue checklist is the tracking document for the PPE issue: the item, the size, the date of issue, the recipient, and the return. It is the document that connects the PPE to the person, and the one that answers the question when the investigation asks who was issued the harness, and when. It is the register behind the inspection, and the one that makes the PPE issue a record, not a handover.

Work through it per issue: the item, the size, the date, the recipient, the signature, and the return date where the PPE is returned. The register is the running record, and the export to CSV is the record for your PPE system. The issue checklist is the counterpart to the inspection checklist: the issue is the start of the PPE life, the inspection is the maintenance, and the two together are the PPE record.

How to use the template

  1. Set the header — the site, the date, the issuer
  2. Add one line per PPE issue — the item, the size, the quantity
  3. Record the recipient — the name, the role, the signature
  4. Set the date of issue, and the return date where the PPE is returned
  5. Sign the issue — the issuer and the recipient
  6. Print it, or export to CSV for your PPE register

Worked example

A PPE issue for the new starters: the hard hats, size L, 3 issued to the 3 new operatives, the signatures, the date; the hi-vis, size M and L, 3 issued; the boots, sizes 9 and 10, 3 issued; the gloves, size L, 3 pairs issued. 12 issues, 12 signatures, the CSV exported to the PPE register. The issue is the start of the PPE life, and the register is the record that the inspection works from.

Field use of the PPE Issue Checklist template

Standard & legal context

The PPE issue is the provision duty under the UK PPE at Work Regulations 1992, which require the PPE to be provided free of charge and maintained, and the OSHA 1910.132 provision duty in the US. The ISO 45001:2018 clause 8.1.2 and the PPE within the controls carry the duty at the management system level. The issue checklist is the record of the provision, and the register is the audit trail the investigation follows.

Common mistakes

  1. No signature on the issue — the signature is the acknowledgement, and the issue without it is the one the investigation questions
  2. No size recorded — the size is the fit, and the PPE that does not fit is the PPE that does not protect, and the size is the record
  3. No return date — the PPE that is returned is the PPE that is inspected on return, and the return date is the trigger
  4. No register — the issue is the start of the PPE life, and the register is the record the inspection works from

Common questions

PPE Issue Checklist — answered

What is a PPE issue checklist?

The tracking document for the PPE issue: the item, the size, the date of issue, the recipient, the signature, and the return date. It is the document that connects the PPE to the person, and the register behind the inspection.

How is the issue checklist different from the inspection checklist?

The issue checklist is the start of the PPE life — the provision and the tracking. The inspection checklist is the maintenance — the condition and the service life. The two together are the PPE record: the issue is the start, the inspection is the maintenance.

Is the PPE issue checklist free?

Yes — print as many as you need, sign them, and file them. Nothing is uploaded and there is no account. The checklist is the standard HSE document, and the template is ready to use.

Does it cover the return of the PPE?

Yes — the return date is a field, and the PPE that is returned is the PPE that is inspected on return. The return date is the trigger for the inspection, and the two checklists connect at the return.

Can I use it in the UK and the US?

Yes — the UK carries the PPE at Work Regulations 1992, the US the OSHA 1910.132 provision duty. The checklist is the issue structure, and the standard section names both. The provision duty is the same, and the record is the same.

Can I print or export it?

Yes — Print / Save as PDF gives the issue as a dated record, and Export CSV gives the items, the sizes, the recipients and the dates for your PPE register. The completed checklist is the document for the file and the one the investigation follows.

Safety disclaimer

These templates are practical documents, built to the UK baseline with the US equivalents named on each page. They do not replace a competent person assessment for high-risk work, a statutory assessment, or the advice of your insurer. Where a document has legal force — the fire risk assessment, the COSHH register, the permit to work — the responsible person owns it. Use the template as the preparation, and the competent person as the sign-off.
