Near Miss Reporting Form
The near miss reporting form is the document for the near miss: the event, the potential outcome, the immediate action, and the cause. It is the form the worker fills in when the incident did not happen, and the one that feeds the near-miss register, the leading indicator that predicts the incident. The near miss is the free incident, and the form is the document that makes the free incident a record, and the record an action.

Near-Miss Reporting Form + the running log
Report the miss, press the button, and it lands in the log with a running tally. The log is the record; the tally is the metric.

| Date | Location | Type | What happened | Cause | Reporter |

Work through it per near miss: the date and the location, the type, the what-happened, the potential outcome, the immediate action, and the cause. The potential is the point, because the potential is what sets the priority, and the form that does not name the potential is the one that gets no action. Add the line to the register, and the form and the register are one document. Print it, or export to CSV for your near-miss system, and the trend of the near misses is the trend the management review reads first.
How to use the template
- Set the event details — the date, the location, the type, the reporter
- Write the what-happened — the one factual paragraph, the facts not the interpretation
- Name the potential outcome — the injury or the damage that was the close call
- Record the immediate action — what stopped it, what was done right away
- Record the cause, and add the line to the near-miss register
- Print it, or export to CSV, and review the trend at the management meeting
Worked example
A near miss, 14:20, at the dock: the type the vehicle, the what-happened the forklift reversed and clipped the pallet stack and the pedestrian stepped back, the potential the struck by, serious, the immediate action the area cleared and the forklift parked, the cause the route crossed the forklift line. The line in the register, the trend the vehicle cluster, and the action the barrier at the crossing. The near miss is the free incident, and the form is the document that makes the free incident the action that prevents the real one.
Standard & legal context
The near miss report is the leading indicator for the UK MHSVR 1999 and the HSE practice, and the OSHA incident and near-miss review in the US. The ISO 45001:2018 clause 10.2 investigation of the incidents including the near misses carries the explicit duty, and the clause 9.1 monitoring carries the leading indicator. The near-miss ratio is the metric, and the form is the document that feeds the ratio, and the register is the data the ratio is built from.
Common mistakes
- Reporting only the injuries — the near miss is the leading data, and the register with only the recordables is the lagging indicator
- No potential named — the potential is the judgement that sets the priority, and the form without it is the one that gets no action
- The immediate action without the cause — the mop is the immediate, and the drain is the cause, and the form carries both
- No register line — the form is the report, and the register is the trend, and the form without the line is the one that changes nothing

Common questions
Near Miss Reporting Form — answered
What is a near miss reporting form?
The document for the near miss: the event, the potential outcome, the immediate action and the cause. It is the form the worker fills in when the incident did not happen, and the one that feeds the near-miss register, the leading indicator.
How is the near miss form different from the incident report?
The incident report is for the event that happened, the injury or the damage. The near miss form is for the event that did not happen, the close call. Same structure, different outcome, and the near miss is the free incident, and the form is the document that makes the free incident a record.
Is the near miss reporting form free?
Yes — print as many as you need, fill them in, and file them. Nothing is uploaded and there is no account. The form is the standard HSE document, and the template is ready to use with the register line and the CSV export.
Does the form feed the near-miss register?
Yes — the form carries the register line, so the report and the register are one document. The form is the report, the register is the trend, and the two together are the near-miss system.
Is the near miss reportable to the HSE?
Most near misses are not reportable to the HSE under RIDDOR, but the dangerous occurrences are the reportable near-miss-equivalents. The other near misses go to the register, and the register is the ISO 45001 clause 10.2 evidence, and the leading indicator the management review reads.
Can I print or export it?
Yes — Print / Save as PDF gives the near miss as a dated record, and Export CSV gives the events, the types, the potentials and the actions for your near-miss register. The completed form is the document for the file and the one the management review works from.

These templates are practical documents, built to the UK baseline with the US equivalents named on each page. They do not replace a competent person assessment for high-risk work, a statutory assessment, or the advice of your insurer. Where a document has legal force — the fire risk assessment, the COSHH register, the permit to work — the responsible person owns it.
